File a defect complaint · Independent custody
Document the issue. Preserve the evidence.
Generate a unique case identifier and email address. CC it on every email you send to regulators, dealers, and manufacturers — your correspondence is preserved as part of an independent archive of EV defect documentation.
How this works: When you submit a complaint to a regulator or manufacturer, copy the unique email address generated below into the CC field. Every reply you receive — including templated denials — is preserved. Patterns across many owners' cases may help identify systemic defects that no single complaint can reveal alone.
What this isn't: EV Risk Index does not file your complaint for you. You remain the complainant. We are the custodian of the documentation.
Who should use this
This system is intended for EV owners documenting:
- Unresolved safety concerns or repeated repair failures
- Battery thermal events, range loss, or propulsion shutdowns
- Charging system failures or recurring software defects
- Manufacturer or dealer correspondence that has gone unanswered
- Regulator complaint follow-ups

What to do next
- Save your case identifier and email immediately — write them down or save them in a password manager. They are shown only on this page.
- Open the email template above, paste it into your own email client, customize the description of your issue, and add your name.
- Send the email to the regulator and manufacturer entity listed above, with your case email in CC.
- When the recipient replies, use Reply-All to keep your case email in CC. The full thread is then preserved automatically — no manual forwarding needed.
- Keep a copy of your sent email in your own records as backup.
Professional & Research Access
EV Risk Index is currently in its early preservation and intake phase. Complaint records, correspondence timelines, and manufacturer response patterns are actively being collected and organized across supported jurisdictions.
Verified legal professionals, journalists, researchers, and consumer advocacy organizations may register for early access to receive updates as investigative datasets, verified complaint clusters, and structured exports become available.
Access to identifiable records is restricted and subject to user consent, professional verification, and applicable legal requirements.
EV Risk Index is an independent documentation and preservation platform. We do not provide legal advice, regulatory representation, or warranty adjudication services. Submitting or preserving correspondence through EV Risk Index does not guarantee regulatory action, manufacturer response, recall issuance, reimbursement, or legal outcomes.
Please retain your case identifier for your records. Future account-based retrieval and case-management features may be introduced as the platform evolves.
For early-access registration, contact legal@evriskindex.com with your professional affiliation and area of investigation.
